Matchmaker Forms
Don’t see your loveable new companion animal in our Adoptable Pets section?
You may choose to fill out one of our companion animal Matchmaker Forms. These questionnaires are a simple and easy way for our Adoption Specialists to contact you when an animal is brought into Woods that meets your criteria. You will then be the first to know about specific dogs or cats before they are released for public viewing.
Matchmaker forms can be downloaded by clicking on the links below. After you have printed out the form, please fill it out completely (the more information that you give us, the more likely we will be able to find your PERFECT MATCH) and return it to Woods Humane Society by mail, fax, or drop it by the shelter in person. Please be advised:
- Matchmaker Forms will be kept on file for 6 months
- We do not accept Matchmaker Forms for puppies or kittens
- You must be at least 18 years old to fill out the forms (if you are a child, please have your parent/guardian fill out and turn in the form with you)
- Please understand that Woods Humane Society takes our animal adoptions very seriously. We ask that you carefully consider every aspect and need that adopting a companion animal includes prior to turning in the matchmaker forms. The Woods staff will diligently review these forms, carefully considering every animal at the shelter.
